Social Counselor (Full-Time)
Marshall County Health Department
Requirements:
Graduation from an accredited college or university with a bachelor’s degree AND one year of professional experience in one or a combination of the following: health care for children or adolescent populations; social work; or social or psychological counseling, including, but not limited to, mental health, correctional, or behavioral rehabilitation counseling; or one year of experience in early intervention and/or service coordination with individuals with disabilities in an educational, social services or medical setting. Ideal candidate is bi-lingual in Spanish/English.
Job description:
Provide care coordination for pregnant and postpartum women/adolescents, children from birth to 21, and Children with special health care needs form Birth to age 21. Service may be provided in a variety of settings, i.e.: office, home, or a predetermined location. Provide a comprehensive screening and assessment with the family to determine their needs/pathways of care. Provide education and link families to community resources to address their needs. Communicate care plans and goals in addition to proactively track patients as they go to and from clinical care to communities. Plan and participate in community outreach events to promote services and provide information to targeted populations.
